The action takes place in a depressing-looking hospital with no electricity, where the player's character works as a security guard. An angry-looking old couple roam the hospital, and the player's job is to turn on the electricity and learn the terrible secrets of these two old people.
There is no separation into locations - you can move around the entire hospital, but you can't get out of it. There are many items that can either be applied to anything in the game, or used as tools or weapons.
